a charitable donation on your behalf.
We would like to give you the opportunity to designate a portion
of the total donation to your choice of one or more of a selected
five charities. |

You may choose any or all five charities listed to the right.
Each of the selected charities will receive a portion of the
charitable donation. Simply follow the instructions below.
1. Select the charity or charities of your choice by checking
the box beside each. You may select more than one.
2. Enter your redemption code, located on the back of your Holiday
Charity Gift Card, in the field below the list of charities.
3. Click the Submit button.
4. A confirmation page will automatically pop-up on your screen. |
